GUIDELINES for the

“OATH TAKING CEREMONY OF NEW NURSES”

(August 29, 2009, 1:00PM-5:PM at Davao Christian High School Auditorium, V. Mapa St., Davao City)

1. Registration will be on August 7-24, 2009, STRICTLY no onsite registration.

2. Registration fee will be received at the PNA office located at KIDNAR BLDG., 17-A GEN. LUNA ST., DAVAO CITY.

P400.00 -membership fee

300.00 -oath taking fee

P750.00 -total fees

3. LADIES: white gala uniform with school pin, nurses cap, white duty shoes & stockings,

hair not touching the collar, and without earrings. Light make-up is encouraged.

GENTLEMEN: white gala uniform with school pin, white shoes/duty shoes, haircut, no beard,

mustache, and goatee.

4. Corsage is not allowed.

5. Call time is 1 hour before the actual time of the program.

6. No escort is allowed as the venue is relatively small.

7. The Deans and the PNA officers will check that you are in proper attire for the occasion.

8. Please BRING your GATE PASS. No GATE PASS, no entry.
